Fasting insulin measures the hormone your pancreas releases to move glucose out of your bloodstream and into cells. It often rises years before blood sugar does, which makes it one of the earliest available signals of metabolic change.

Ranges vary between labs, so compare to the range on your own lab report. The standard range is wide, and many clinicians consider values in the upper half worth attention even though they fall inside it.

What fasting insulin measures

When you eat, your pancreas releases insulin to move glucose from your blood into muscle, liver, and fat cells. Measured after an overnight fast, insulin reflects how much of that hormone your body needs at rest to keep blood sugar stable.

That baseline is the useful part. If cells respond well to insulin, very little is required. If cells have become resistant, the pancreas compensates by producing more, and fasting insulin climbs while glucose stays normal. This is why insulin frequently detects insulin resistance well before a fasting glucose or A1c would.

What high or low levels mean

High fasting insulin most often reflects insulin resistance, where cells respond poorly and the pancreas compensates with higher output. It is associated with excess body fat, physical inactivity, poor sleep, and metabolic syndrome, and it commonly precedes prediabetes and type 2 diabetes by years. In women it is also relevant to polycystic ovary syndrome.

Low fasting insulin is usually unremarkable in someone with normal blood sugar and can simply reflect good insulin sensitivity. Low insulin paired with elevated glucose is a different picture and suggests the pancreas is not producing enough, which requires clinical evaluation.

Insulin is most informative read next to fasting glucose rather than alone, since the two together distinguish good sensitivity from compensation.

My glucose and A1c are normal but my insulin is high. What does that mean?

This is the pattern insulin testing exists to catch. It suggests your cells have become resistant and your pancreas is working harder to hold blood sugar in the normal range. Glucose stays normal until that compensation starts to fail, which can take years.

Do I need to fast for this test?

Yes. Insulin rises sharply after eating, so a non-fasting result cannot be interpreted against the reference range. Aim for 8 to 12 hours without food, water is fine.

Why isn't this on every standard blood panel?

Most routine panels check glucose and A1c, which detect problems once blood sugar has already shifted. Insulin is a more forward-looking marker, and it is increasingly ordered when the goal is early detection rather than diagnosis.
